8. Certificate of trainings/seminars attended, arranged according to date from recent to past;

    

9. Published research relevant to the profession, if any;

10. Awards and citations received, if any;

      

11. Other professional achievements (within the year) (e.g. Resource speaker, facilitator, coordinator of related

      professional activity, officer of professional civic organization), if any;

12. Updated PRC ID/License;

                

13. Updated PHIC Accreditation;

          

14. Diplomate certificate, and

15. Fellow certificate, if any.
